Notice of Privacy Practices
This notice describes how health information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Our commitment
Tula Care is required by law to maintain the privacy of protected health information, to give notice of our legal duties and privacy practices regarding that information, and to notify affected individuals following a breach of unsecured protected health information. We are required to follow the terms of the notice currently in effect.
How we may use and disclose health information
The full notice describes each of the following in detail, including the limits that apply to mental health records under Oregon law:
- Treatment — sharing information with the professionals involved in your care.
- Payment — obtaining reimbursement for services provided.
- Health care operations — quality review, staff training, and licensing oversight.
- Uses and disclosures required by law — including mandatory abuse reporting and responses to lawful requests from regulators.
- Uses and disclosures requiring your written authorization — including most uses of psychotherapy notes, marketing, and any sale of information.
Your rights
- To inspect and get a copy of your health record.
- To ask us to correct information you believe is wrong or incomplete.
- To ask for confidential communications — for example, contact at a specific phone number.
- To ask us to limit what we use or share, and to restrict disclosure to a health plan for care you paid for yourself.
- To get a list of disclosures we have made.
- To get a paper copy of this notice, even if you agreed to receive it electronically.
- To choose someone to act for you, such as a legal representative or guardian.
Complaints
You may complain to Tula Care, or directly to the U.S. Department of Health and Human Services Office for Civil Rights, if you believe your privacy rights have been violated. You will not be retaliated against for filing a complaint — not by us, and not in any way that affects your care.
